KVKK Application and Information Request Form
GENERAL DESCRIPTIONS
Personal Data Owners (hereinafter shall be referred to “Application Owner”) being defined as relevant person in personal data protection act (“KVK”) Nr.6698, have been furnished with the right of requesting certain demands in connection with processing personal data in article 11 of KVK Act.
Applications in connection with aforesaid rights to be submitted to our company by data responsible under the article 13/i of KVK should be conveyed to our company either in writing or by means of other methods having been defined by Personal Data Protection Board (“Board”).
The fact that personal data we hold about you should be correct and updated is important. Therefore, please inform us if any change would occur in your personal data. under KVK and relevant legislation, you, in connection with yourself, may;
- Learn whether personal data is processed or not,
- Demand details in connection with data we collected,
- Learn, what reasons these data are collected for and how they are used, as well as the third persons within country or abroad to whom such information are transferred, or
- Desire correcting, updating such information if they are processed imperfect or wrong; deletion of the same if the reasons for processing such information are already lifted including notifications to be submitted to the third persons to whom such information have been transferred.
In frame of aforesaid, applications to be submitted to our company in “writing” together with extracted copy of this form and upon writing “personal data protection act – request for information” on the envelope or in subject line, and shall be conveyed to us ;
- By means of personal application of application owner,
- By means of notary or registered mail with return receipt,
- By sending registered electronic mail address of our company.

Your application shall be answered in shortest time possible but with 30 days latest under the provisions of KVK. In the event that information and documents you would present would be incomplete or incomprehensible, we in quality of data responsible shall contact with you in order to finalize your application.
